Etiqa Family Takaful Berhad/Etiqa General Takaful Berhad (the Company), formerly known as Etiqa Takaful Berhad, is a public limited liability company, incorporated and domiciled in Malaysia. On 1 January 2018, the MAHB Group has successfully completed the conversion of composite licenses to single licenses (license split) and surrendered the composite licenses in exchange for the four single licenses. NZ Network Management Services Sdn Bhd is an authorized Corporate Agency of Etiqa Family Takaful Berhad / Etiqa General Takaful Berhad appointed to manage UM International students’ medical plan.

The coverage will continue as long as you are registered UM student, paid the takaful contribution and subject to renewal of the plan made between UM and Etiqa Family Takaful Berhad / Etiqa General Takaful Berhad.

Etiqa is going cardless. You no longer need a medical card for hospital admission. Just present your identification (passport/iKad) and inform the hospital you are covered with Etiqa. The hospital will then perform verification on our Portal and process your Guarantee Letter (GL) request.

Meanwhile, for outpatient treatment in General Practitioner clinic (GP), the treatment would be on ‘pay and claim’ basis. Outpatient sickness in specialist will not be covered. You can also read the outpatient exclusions on our website.

Sickness, disease or illness which you have been suffering from prior to the effective date of your cover.

Government Hospital do accept Guarantee Letter issued by Etiqa. For inpatient treatment in any Government Hospital, you need to get the “pre-admission/pre-authorization” form from us and the form needs to fill in by the treating doctor. Then, you need to email the completed form to etiqahealthcare@etiqa.com.my
You can also call Etiqa HealthCare Call Centre, 1800 88 9998 to request Guarantee Letter by giving detailed information regarding treatment/disease/hospital etc.

Yes, this medical plan does cover a normal outpatient treatment in any GP clinic subject to a maximum limit of RM500 per annum. Please pay the bill first and then submit the original receipt to us for reimbursement.

Please ensure the room rate per day is RM200.00 or below. If the rate is higher than RM200.00, the patient shall bear 20% of the eligible benefits.

Members only have to give name and passport no and the hospital will verify with Etiqa Health Care Centre.

Outpatient treatment (OP) refers to outpatient visit at GP clinic only without being hospitalized. Inpatient treatment refers to admission in hospital (at least 1 night stay).

Refers to outpatient treatment at a normal clinic other than Private Hospital or specialist clinic located in Private Hospital.

No. All outpatient treatment at GP clinic is on pay and claim basis (reimbursement) except for treatment at UM Student Health Clinic (KKPUM).

Any medicines purchased at pharmacy is NOT claimable.

Not covered except for accidental caused only.

This plan does not cover any medical cost related to pregnancy treatment.

If you reach your outpatient limit, it means you have utilized the maximum coverage amount provided by your insurance policy. We will typically stop covering any additional medical expenses and you will be responsible for paying for any further treatments or services out of pocket.

This is a Group policy. Hence, no refund shall be made to un-claim members.
