HOW TO APPLY

Method 1 – On-line Registration

Step 1 : Please refer to “Insurance Plan” for detail coverage
Step 2 : Choose your plan either Plan 1(RM350) or Plan 2(RM500)
Step 3 :  

Remit your insurance premium according to your chosen plan via :-

  • Internet banking (CIMB click, Maybank2u etc.)
  • Any CIMB bank branches or ;

To the following Bank’s account :-

Bank’s Name: CIMB Islamic Bank Berhad
Account Name : NZ Network Management Services Sdn Bhd (Etiqa Takaful Corporate Agency)
Account No : 8602 323 535

(Please retain the deposit slip/ receipt as a proof of payment)

Step 4 :  Fill up/ Complete the “Application Form” at APPLY NOW and attached “proof of payment” above
Step 5 :  Click “Apply Now” – End successfully.
Step 6 :  Please collect your receipt and Insurance Cover Letter (for visa/student pass purposes) at Etiqa Takaful Agency Office, Ground Floor, Block E Perdanasiswa Complex, Universiti Malaya 50603 Kuala Lumpur

 

Method 2 – Walk-in Registration

Step 1 : Fill up / complete the manual Application Form and make a payment over the counter at Etiqa Takaful Agency Office, Ground Floor, Block E Perdanasiswa Complex, Universiti Malaya 50603 Kuala Lumpur.
Step 2 : Please obtain the receipt and Insurance cover letter from the counter staff.

 

COLLECTION OF MEDICAL/INSURANCE CARD

The medical card will be ready AFTER 14 working days from registration date. We will notify you via email once the card is ready for collection. The card can be collected at the above address. This medical card is required for ADMISSION in private hospital only.
